Office hours and after-hours policy
Our hours of operation are from 8:00 AM-5:00 PM Monday-Thursday and 8:00 AM-12:00 PM Friday. On-call physicians are available at night and on weekends. In case of an emergency, please call 911.
Prescription refills and preauthorization
Bring an updated list of medications to each visit and ask for refills. If you run out of medications at home please contact your pharmacy directly and allow 24-48 hours for a refill. Should your insurance carrier require preauthorization for your prescription, a $25 fee will be charged for the time required to complete the necessary forms.
Late arrivals and missed appointments
Please be on time to your appointment and call if you are running late. We will try to accommodate you but may need to reschedule if you are not on time. You may be charged for a missed appointment if a 24-hour notice is not given.
We would appreciate a 24-hour notice if you need to cancel your appointment.
We work with many insurance providers and plans. Proof of insurance must be presented for all services provided. If proof of insurance is not provided, or we are not contracted providers with your insurance plan, your account will be considered a cash account, with full payment expected at the time of service. Our billing office is happy to help you determine your payment obligations before service is provided. To prevent billing or communication errors, you will be asked to update or confirm your registration at each visit. If you change insurance between visits, please contact us.
